For me to have the right to participate in any/all KD’S Husky Rescue AZ, I hereby release KD’S Husky Rescue AZ, and any of its volunteers and agencies from all liabilities, claims, demands, causes of action, loss, damage, or injury to person or property including any death and or serious injury which may result while preparing for or participating in a KD’S Husky Rescue AZ event. I understand that participation in outdoor activities or activities related to husky handling is physically demanding, and I hereby verify that I am physically able to engage in this activity. I am aware of the risks associated with dogs, especially rescue dogs. I hereby release KD’S Husky Rescue AZ from any claim whatsoever which may arise because of any first aid, treatment, service, or assistance provided to me in connection with any injury that arises from volunteering or participating in activities associated with this rescue. I have read this waiver and release of liability and understand that it is a full and complete unconditional release form and waiver of liability and indemnity for any injury that I suffer. By signing this release, I give up all rights to litigation.Required, you will not be allowed to volunteer without this consent.Veterinary Care (Regular and emergency care)(Required) I agree that if any medical issues come up, to contact KD’S HUSKY RESCUE AZ before taking the dog in my care to a vet for treatment – unless it is a life-threatening emergency. If it is a life-threatening emergency I will seek out care immediately and contact KD’S HUSKY RESCUE AZ as soon as possible, an attempt to contact should still be made beforehand.We partner with local vets and prefer to have the treatment done there. If it’s a life-threatening emergency and you cannot contact us we understand).Transportation(Required) While transporting a dog for KD’s HUSKY RESCUE AZ, the dog must be equipped with an approved collar and/or harness with tags identifying the rescue. The dog must be on a leash that is also pre-approved and without damage. The dog should be in a crate or kennel pre-approved by the rescue when possible.
